29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 29 November 2018, and at times prior thereto, two framers were installing siding on a residence under construction without the use of head protection, exposing the workers to the hazard of being struck to the head by falling tools, materials, or debris.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 29 November 2018, and at times prior thereto, a framer was using a pneumatic nail gun without eye protection, exposing the worker to the hazard of being struck to the eye or face by flying projectiles or particles.

On or about 29 November 2018, and at times prior thereto, a framer used an extension ladder to gain access to the first floor top plate without the ladder being extended at least 3 feet above the upper landing, exposing the worker to a fall hazard of approximately 9 feet 10 inches.
