29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift: On or about November 29, 2018, employee was replacing the fascia on an apartment complex building while operating an aerial lift without the use of fall protection equipment. This condition exposed employee to falls in excess of 10 feet.
