29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    Core #5:    Employer did not develop and implement procedures that outlined specific procedural steps to isolate equipment to control all hazardous energy sources and testing equipment to ensure that isolation was effectiveness of energy control for employees who perform maintenance on the Fractionator Distillation Process Units.       On or about November 24, 2018, employees who were troubleshooting and removing an electrical centrifugal pump downstream from the I-100 fractionator column did not isolate the re-boiler loop line that fed back into the distillation column causing a leak of vegetable grade palmitic fatty acid (product #TRV1695) that was at approximately 500 degrees Fahrenheit and under a vacuum. As the TRV1695 leaked out of the open pump, it ignited resulting in a large conflagration.
