29 CFR 1910.36(e)(2): The door that connects any room to an exit route must swing out in the direction of exit travel if the room is designed to be occupied by more than 50 people or if the room is a high hazard area (i.e., contains contents that are likely to burn with extreme rapidity or explode). In the warehouse: On or about November 29, 2018, the exit door located in a high hazard area that was likely to burn with extreme rapidity was swinging to the inside against the direction of exit travel. This condition exposed employees to fire hazards and entrapment.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: In the warehouse: On or about November 29, 2018, the clear working space of 3- feet on the electrical panels located in the warehouse and the immediate stage area were not maintained. This condition exposed employees to fire and electrical shock hazards.
