29 CFR 1910.23(c)(4): Ladders were used on stable and level surfaces, but were not stabilized to prevent accidental displacement: East side of commercial building. On or about November 27, 2018, employees were exposed to a fall hazard while accessing the roof from a non-self-supporting extension ladder which was used at an angle such that the horizontal distance from the top support to the foot of the ladder was greater than one-quarter of the working length of the ladder.

29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: East side of commercial building. On or about November 27, 2018, employees were exposed to a fall hazard of greater than 4 feet while accessing the HVAC components without using fall protection.
