Employees were exposed to an unapproved front end attachment on a fork lift. The employer did not ensure that the addition of work platform would affect the safe operation of the equipment. The work platform in use is not an approved attachment per the fork lift manual. Serial number of the fork lift that employer uses is 0160019807.

[44 FR 8577, Feb. 9, 1979; 44 FR 20940, Apr. 6, 1979, as amended at 58 FR 35183, June 30, 1993; 63 FR 66274, Dec. 1, 1998] 29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: At a construction site located at 17601 E 32nd St Independence MO 64050 employees were exposed to an operator of forklift that had not been trained in the last three years. The employer operated a fork truck without having the operators performance being conducted at least once every three years. The last certification was conducted in 2010.
