At a construction site located at 17601 E 32nd St Independence MO 64050, employees were exposed to contact with hazardous substance.  The employer did not ensure employees wore appropriate eye protection when exposed to eye hazards from the substance Ash Grove Pre mix Stucco.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:        At a construction site located at 17601 E 32nd St Independence MO 64050, employees were exposed to fall hazards.  Employees were exposed to an unapproved front end attachment on a fork lift.  The employer did not ensure that the addition of work platform would affect the safe operation of the equipment.  The work platform in use is not an approved attachment per the fork lift manual.  Serial number of the fork lift that employer uses is 0160019807.
