29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: The employer is failing to ensure employees are protected from fall hazards when conducting cleaning and repairs as part of maintenance operations. Employees do cleaning and maintenance work on the top of the deck of the iron grinder without utilizing fall protection. This was most recently documented at 2400 G St., South Sioux City, Nebraska on or about November 28, 2018 where employees are exposed to falls of 19 feet to concrete below.
