29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of particulates not otherwise regulated, total dust, listed in Table Z-1 in excess of the permissible exposure limits as 8 hour Time Weighted Average concentration of 15 milligrams per cubic meter of air..    (a)     The welder working in the NW corner of the Plant 2 - Weld area was exposed to particulates not otherwise regulated - total dust at an 8-hour time weighted average of 25.40 milligrams per cubic meter of air, approximately 1.7 times the limit of 15 milligrams per cubic meter of air.  The sample was collected on January 16, 2019 during a 441 minute sampling period and exposure calculations include a zero increment for the 39 minutes not sampled.      (b)     The welder working in the NW corner of the Plant 2 - Weld area was exposed to particulates not otherwise regulated - total dust at an 8-hour time weighted average 23.04 milligrams per cubic meter of air, approximately 1.5 times the limit of 15 milligrams per cubic meter of air.  The sample was collected on January 16, 2019 during a 460 minute sampling period and exposure calculations include a zero increment for the 20 minutes not sampled.    Weldall Mfg. Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1070296, citation number 1, item number 2b and was affirmed as a final order on (12/10/2015), with respect to a workplace located at 2001 S. Prairie Ave, Waukesha, WI 53189.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a) On and before January 8, 2019, welders were exposed to airborne contaminants in the workplace including copper fume, iron oxide fume, and particulates not otherwise regulated, total dust, had not been trained on health and physical hazards of the chemicals present in the work area prior to being exposed to hazardous chemicals at the worksite.    Weldall Mfg., Inc. was previously cited for a violation of a similar occupational safety and health standard, which was contained in OSHA inspection number 1070296, citation number 1, item number 5b and was affirmed as a final order on (12/10/2015), with respect to a workplace located at 2001 S. 29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s)    (a) On and before November 26, 2019. The employer did not maintain SDS sheets for commonly used metals in Plant 2, such as A36, A42, A992 on the electronic SDS database that was to be used as a the primary method for employees to reference the hazardous nature of chemicals onsite. This exposes employees to chemical exposure hazards and prohibits employees from accessing SDS data regarding health effects from chemicals that they are exposed to and emergency response actions in the case of an emergency.    Weldall Mfg., Inc. was previously cited for a violation of a similar occupational safety and health standard, which was contained in OSHA inspection number 1070296, citation number 1, item number 5b and was affirmed as a final order on (12/10/2015), with respect to a workplace located at 2001 S.
