29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 30, 2018, the employer failed to ensure that the Kingston Heavy Duty 30 Decap lathe (W0208), which was being used in the Decap department, was effectively guarded to protect employees from the hazard of point of operation and flying chips during the operation of the lathe. The lathe did not have a chuck guard in place.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: On or about November 30, 2018, the employer failed to ensure the compressed air hoses used at various lathes throughout the building, to include but not limited to the Kingston Heavy Duty 30 lathe (W0159) that is located in the Group 2 area, was designed such that it was able to control the flow of air to a maximum 30 psi. The hose was tested onsite and registered a reading in excess of 70 psi. The air hoses are used to clean off the lathe, miscellaneous parts, and other debris.
