29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      a)  Line 1 extrusion machine;  On or about November 27, 2018 and at times prior, the employer exposed employees to the hazardous energies, including but not limited to electrical and pneumatic, present on machinery in that the employer failed to develop and document machine specific energy control procedures to protect employees while employees were required to perform servicing and maintenance tasks.    b)  Line 2 extrusion machine;  On or about November 27, 2018 and at times prior, the employer exposed employees to the hazardous energies, including but not limited to electrical and pneumatic, present on machinery in that the employer failed to develop and document machine specific energy control procedures to protect employees while employees were required to perform servicing and maintenance tasks.    c)  Line 3 slit film extrusion machine; On or about November 27, 2018 and at times prior, the employer exposed employees to the hazardous energies, including but not limited to electrical and pneumatic, present on machinery in that the employer failed to develop and document machine specific energy control procedures to protect employees while employees were required to perform servicing and maintenance tasks.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:       a)  Extrusion Lines;  On or about November 27, 2018 and at times prior, the employer exposed his employees to the hazardous energies present on machinery in that employees were required to perform maintenance and servicing on the extrusion lines without these employees being provided with the level of training required that ensured that the purpose and function of the energy control program and the knowledge required for the safe application, usage and removal of the energy control device(s) was acquired.
