29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.    On or about November 28, 2018, at a sign replacement site located at 4301 N. Western in Oklahoma City, OK 73118, an employee was allowed to work in the basket of an aerial lift without fall protection, exposing the employee to the hazard of falls.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     On or about November 22, 2018, at a sign replacement site located at 4301 N. Western in Oklahoma City, OK 73118, an employee was allowed to work on a roof without fall protection, exposing the employee to the hazard of falls.
