This condition was observed on or about November 30, 2018.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) 2825 Mercer St., Philadelphia, PA 19134 - The employer did not ensure that an excavation was protected from cave-ins by an adequate protection system while an employee worked in the excavation, which was approximately 6 feet 10 inches, thus exposing the employee to cave-in/between hazards. This condition was observed on or about November 30, 2018. RPS Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1305151, citation number 1, item number 2 and was affirmed as a final order on September 6, 2018, with respect to a workplace located at 441 Dudley Street, Philadelphia, PA 19148.
