29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  JOB SITE: 13825 Park Steed Drive, Earth City, MO 63045, on or about 11/28/2018, the employer failed to establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on equipment, but not limited to the Polar MOHR Cutter, Model: 115CE, S/N: 47316692, and IXP Cutter, Model: 24, S/N: 850052.   The workers were exposed to an electrical hazard.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  JOB SITE: 13825 Parks Steed Drive, Earth City, MO 63045, on or about 11/28/2018, employees were permitted to use a 110v 4-receptacle electrical metallic box with knock and punch outs as an electrical extension cord in the production area of the building.  The workers were exposed to an electrical hazard.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  JOB SITE: 13825 Parks Steed Drive, Earth City, MO 63045, on or about 11/28/2018, workers were provided with a portable vacuum cleaner to clean out the IXP 24 Cutter daily, which was missing a ground prong on the power cord.  As a result, the path to ground from the equipment was not permanent, continuous, and effective.  The workers were exposed to an electrical hazard.
