29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 29, 2018, employees were conducting framing activities on the second floor of the residential property under construction and were not protected by a fall protection system, exposing employees to a fall hazard over 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about November 29, 2018, employees working on the residential property under construction had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazard of approximately 10'5" feet.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about November 29, 2018, employees were using the wood frame of the residential property under construction to gain access to the second floor, exposing employees to a fall hazard of approximately 10 feet.
