29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:       On or about November 26, 2018, employees were exposed to serious physical injuries during maintenance and servicing operations on barrel machines, and the employer did not provide safety training to ensure employees understood the purpose and function of the companys energy control program.  This safety training would include the following:      a. Authorized Employees - training in recognition of applicable hazardous energy sources, the type and magnitude of the energy available, and the methods and means necessary for energy isolation and control;   b. Affected Employees - training in purpose and use of the energy control procedures; and   c. All Other Employees - instructed about the procedure and prohibition relating to attempts to restart or reenergize machine or equipment which are locked or tagged out.

29 CFR 1910.147(d)(4): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about November 30, 2018, an employee was exposed to amputation hazards associated with ingoing nip points and rotating parts on the #3 Barrel Machine, during maintenance and servicing operations. The employer failed to implement energy control application steps.  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:     On or about November 26, 2018, an employee was exposed to serious physical injuries from an unguarded rotating drive shaft roller on the underside of the #9 Barrel Machine, as the employee cleaned debris and scrap paper from beneath the machine.
