29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On January 16, 2019, Edwyn Salador Jimenez was issued citations and the employer did not document that each citation was abated.  Abatement documentation was not received for the following violations:       a.    Citation 1 Item 1, and  b.    Citation 1 Item 2

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 29, 2018, an employee was exposed to struck-by hazards on the head without head protection, while working under an employee at a higher work level, on a residential construction site in Mahomet, Illinois.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about November 29, 2018, an employee was exposed to fall hazards of 10.5 feet without fall protection, on a residential construction site in Mahomet, Illinois.
