29 CFR 1910.178(l)(1)(i): Operators were not trained in the safe operation of powered industrial trucks: (a) Green Roofing & Construction, LLC at 11801 York St., Northglenn, CO 80233: On and before November 27, 2018, the employer did not ensure all forklift operators were trained in operation of powered industrial trucks in that the operators training consist of a combination of formal instruction, practical training, and evaluation of the operators performance in the workplace. At least one of the operators training consisted of on-the-job, self-taught training only and was also not certified. This condition exposed employees to the hazard of unsafe fork-lift operations.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Green Roofing & Construction, LLC at 11801 York St., Northglenn, CO 80233: On and before November 27, 2018, the controlling employer did not ensure the subcontractor employees were protected from injury, in that approximately 48 employees were engaged in re-roofing activities on 10 large apartment buildings, two or three stories high, without the use of a fall protection system. This condition exposed the employees to fall hazards of approximately 18-27 feet.
