29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)   PIA Construction LLC at 11801 York St., Northglenn, CO 80233: On and before November 27, 2018, the exposing employer did not ensure that the ladders employees were using to access the upper landing surfaces extended 3 feet above the upper landing areas. Several employees engaged in re-roofing activities on the roof of a 2 story apartment building (#2) did not have their extension ladder adjusted correctly so that the side rails extended at least 3 feet above the work surface. This condition exposed the employees to a fall hazard of approximately 18 feet.
