29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) On or about 11/27/18 - 4437 Cahaba River Road, Vestavia Hills, AL, employees working on the roof engaged in activities such as, but not limited to, installing shingles, ridge vents, and using leaf blowers to clean off debris were exposed to falls of approximately 14 feet, 9 inches.    Roberto Triste-Cruz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1325971, citation number 1, item number 2 and was affirmed as a final order on 09/06/2018, with respect to a workplace located at 1705 Derby Drive SE, Cullman, Alabama, 35055.

(a) On or about 11/27/2018 - 4437 Cahaba River Road, Vestavia, AL, an in-patient hospitalization that occurred on 11/27/2018 was not reported to OSHA until 12/03/2018.
