29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  On or about November 30, 2018, at 10301 NW 50th, Suite 101, Sunrise, FL 33351, the employer allowed its employees to work at a jobsite where the designated Exit doors had locks. Employees can possibly be trapped and exposed to smoke inhalation, burns and possible death.

29 CFR 1910.36(e)(2): The door that connects any room to an exit route did not swing out in the direction of exit travel when the room is designed to be occupied by more than 50 people or if the room is a high hazard area:  On or about November 30, 2018, at 10301 NW 50th, Suite 101, Sunrise, FL 33351, the employer allowed its employees to work in an area where a designated Exit door that connects to an exit route did not swing out in the direction of exit travel. Employees can possibly be trapped and exposed to smoke inhalation, burns and possible death.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  On or about November 30, 2018, at 10301 NW 50th, Suite 101, Sunrise, FL 33351, the employer failed to receive written approval from a forklift manufacturer prior to allowing the operation of a forklift that was used with a front-end elevating platform (man-basket) and not marked with alternative capacities, thus exposing employees to fall and struck-by hazards.
