29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards:                  a) On or about December 4, 2018 at Lot 29 on Preserve Lane in Sheffield Village, OH:  The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) On or about December 4, 2018 at Lot 29 on Preserve Lane in Sheffield Village, OH:  The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On or about December 4, 2018 at Lot 29 on Preserve Lane in Sheffield Village, OH: The employer did not ensure employees were protected from fall hazards while replacing roofing material on a residential type roof structure.  Employees operating in this condition were exposed to approximately a 9 ft. fall to the ground below.    J & J Home Improvements, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1311725, citation number 1, item number 1 and was affirmed as a final order on May 24, 2018, with respect to a workplace located at 408 Herns, Barberton, Ohio.
