29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable:    a. On or about November 20, 2018, employees operating nail guns were not provided eye protection and were being exposed to flying objects.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about November 30, 2018, front left side of home, the 12-foot ladder was extended only 16 inches above the upper landing surface, exposing employees to a fall hazard of 10 feet to the ground.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a. On or about November 30, 2018, on a 6:12 pitch roof, several employees performing roofing work were not protected from falling 10 feet by the use of a guardrail system, safety net system or a personal fall arrest system.      Tier 1 Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1305196, citation number 01, item number 001 and was affirmed as a final order on April 27, 2018, with respect to a workplace located at 10174 Windward Way N, Jacksonville, FL 32256.
