29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 27, 2018, Salvador Alvarado did not ensure his employees were protected from fall hazards while engaged in re-roofing construction on top of a residential home. Measurements taken on the residential home were approximately 20 feet at the height of peak of the house and approximately 16.4 feet where employees were grabbing roofing material and working.

(a) On or about November 27, 2018, Salvador Alvarado did not ensure each employee operating a pneumatic nail gun while conducting residential re-roofing wear proper eye protection to protect from flying particles.
