29 CFR 1926.502(e)(2): Positioning devices shall be secured to an anchorage capable of supporting at least twice the potential impact load of an employee's fall or 3,000 pounds (13.3 kN), whichever is greater.  a. On or about December 4, 2018, located at 600 South High Street in Columbus, Ohio, two employees performing shingle tear-off on a steep-pitched (Approximately 11/12 pitch roof) were connected to a single 3M ridge anchor that was inadequately secured over roofing shingles, thereby exposing the employees to a maximum 30 foot fall hazard.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. On or about December 4, 2018, located at 600 South High Street in Columbus, Ohio, employees used an aluminum portable ladder to gain access to a roof that did not extend at least three feet above the eave, thereby exposing the employees to a fall hazard of approximately 12 feet.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  a. On or about December 4, 2018, located at 600 South High Street in Columbus, Ohio, employees climbed aluminum portable  ladders to gain access to the bank's roof. The ladder was not used for the purpose it was designed when the extension ladder was separated to make two individual ladders to climb onto two separate rooftops, thereby exposing the employees to a fall hazard of approximately 12 to 22 feet.

On or about December 4, 2018, located at 600 South High Street in Columbus, Ohio, the employer did not maintain a safety and health program to ensure hazard detection and correction when performing tear off on a steep-pitch roof and working from ladders, thereby exposing the employees to the hazards associated with roofing work.   Rene Flores was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1299413, citation number 1, item number 1 and was affirmed as a final order on May 29, 2018, with respect to a workplace located at 510 West Town Street Columbus, Ohio 43215.

Rene Flores was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1299413, citation number 1, item number 2a and was affirmed as a final order on May 29, 2018, with respect to a workplace located at 510 West Town Street Columbus, Ohio 43215.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a. On or about December 4, 2018, located at 600 South High Street in Columbus, Ohio, employees were performing roof work without having fall protection when walking a steep-pitched roof to an extension ladder, thereby exposing the employees to a maximum fall hazard of approximately 30 feet.   Rene Flores was previously cited for a violation of this Occupational Safety and Health equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1299413, citation number 1, item number 3 and was affirmed as a final order on May 29, 2018, with respect to a workplace located at 510 West Town Street Columbus, Ohio 43215.
