29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Ground men, employees performed maintenance activities, including the use of open flame torches, while in close proximity to plumbing, processes, pumps, and vessels which contained combustible and potentially flammable liquids and gasses, and were not provided fire resistant pants, and either long sleeved shirt or jacket, on or about, December 6, 2018. ABATEMENT CERTIFICATION REQUIRED
