29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) A & D Iron Works Inc. 305 24th St. Brooklyn, NY 11232. Throughout the facility, employees fabricating the doors, window guards, and/or gates, are exposed to chemical hazards from, but not limited to, paints, welding rods and paint thinners. The employer provided the employees with SAS N95 respirators. However the employer did not develop and implement a written Respiratory Protection Program. The condition was noted at or around 11/30/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
