29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about December 11, 2018, an employee was operating an Okuma LB 15 # 2. The machine door was able to be opened approximately 5 inches before the interlocks engaged. The employee was exposed to rotating parts, flying particles and splashing of chemicals.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.     On or about November 1, 2018 and after, employee(s) were required to perform voltage testing on 240 volts on energized components inside electrical supply boxes.  The employer failed to provide electrical protective equipment appropriate for the specific parts of the body to be protected when they performed such tasks in areas where they had the potential to come into contact with energized equipment or encounter arc flash hazards.    Employees did not wear appropriate electrical protective equipment including, but not limited to, the following to protect against electrical shock injuries:      a) Rubber insulating gloves with leather protectors    b) Arc-Rated clothing such as arc-rated long-sleeve shirt and pants, or arc-rated coverall     c) Hearing protection (canal inserts)     d) Arc-Rated flash suit, hood or balaclava     Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  On or about July 1, 2018, the employer failed during calendar year 2018, to electronically submit the company's 2017 data from their OSHA Form 300A or equivalent by July 01, 2018.  The establishment is classified under NAICS 332911 and employed more than 20 employees during calendar year 2017.  No documentation or certification required.
