Employees perform maintenance, tooling changes and servicing of equipment which includes but is not limited to tape slitter machines and rewind machines.  During maintenance, tooling changes and servicing, unexpected energizing, startup or release of stored energy could occur and cause injury.  The exposed employees to caught-in hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Bron Tapes Inc. located at 875 Ellsworth Ave. Denver, CO 80223:  On and preceding December 3, 2018, the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards present during tape slitter operation.  A WebTec and Lever 500 tape slitter were used to cut rolls of tape into smaller sections with the use of a mandrel that spins the roll as a blade makes cuts. The slitter blade travels parallel to the mandrel on a carriage and makes cuts in intervals specified by the operator.  As the blade contacts the rotating tape rolls, a nip point is created which caught a rag an employee was using to dry the tape roll and pulled the employees hand into the blade.  This condition resulted in a partial amputation of the employees finger.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    a) Bron Tapes Inc. located at 875 Ellsworth Ave. Denver, CO 80223:  On and preceding December 3, 2018, the employer did not ensure that a work-related incident resulting in an amputation was reported within 24 hours. At approximately 9:30 am on 11/19/2018, a worker was cleaning a product mounted on a slitter machine using a rag when the rag became entangled in the blade and pulled her hand into the point of operation. This resulted in an amputation of the employee's finger.  The employer did not report the amputation until December 3, 2018 at 10:30 am.
