29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    1.    On or about November 29, 2018, at Building 1 of the Antigua Apartment job site in Saint Augustine, on the east side edge of the 3rd floor, framing employees installing exterior walls were exposed to a 20 feet fall hazard in that they were not protected by a fall protection system.    2.   On or about November 29, 2018, at Building 1 of the Antigua Apartment job site in Saint Augustine, on the 3rd floor, a framing employee working from the top plate of an interior wall was exposed to an 8 feet fall hazard in that he was not protected by a fall protection system.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    a.   On or about November 29, 2018, at the Antigua Apartments job site in Saint Augustine, on the 3rd Floor of Building 1, a framing employee was exposed to a six feet fall hazard as he used the top step of an A-Frame stepladder to climb up onto the top of the wall.
