29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (a) The employer did not ensure hazardous energy control procedures were created for equipment where servicing and maintenance work is performed, exposing employees to amputation hazards. Crothall Laundry Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1085441, citation number 01, item number 03 and was affirmed as a final order on March 3, 2016, with respect to a workplace location at 9905 S. 13th St., Oak Creek, WI 53154.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources: (a) The employer did not ensure hazardous energy control devices were implemented when conducting servicing and maintenance work on a rail system in the soil department, exposing employees to amputation hazards.
