29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in inpatient hospitalization, amputation or the loss of an eye:    (a) On or about September 17, 2018, the employer failed to report the amputation and hospitalization of an employee who was injured while operating a 1984 Cincinnati Sheet Shear Machine, Model No. 2512G, Serial No. 42803. OSHA was not notified of the incident until November 14, 2018.
