29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) Lower machining area; On or about November 26, 2018, a forklift spreader bar/beam used to lift industrial rollers was not securely attached to the forklift forks. An employee was hospitalized after being struck by a spreader bar/beam (forklift attachment) and load that slid off the forklift forks.
