29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Great Lakes Case & Cabinet, Inc., Forklift battery charging station area, on or about December 7th, 2018 and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, sulfuric acid during the addition of water to the batteries of forklifts onsite. The emergency eyewash station (self-contained) was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that: 1. Weekly visual inspections of the eyewash station and shower were not conducted. The emergency eyewash/shower unit was not activated weekly to verify proper operation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Great Lakes Case & Cabinet, Inc., Assembly/warehouse of the facility, on or about December 7th, 2018 and times prior thereto: Employee utilized a Delta brand horizontal band saw (serial #: RF-8823) that was not equipped with a guard to cover the unused portion of the saw blade and nip points created by the moving saw blade and blade guide rolls.
