29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: A.) FMC Technologies Measurement Solutions, Inc. - On or about 12/6/18, and at times prior thereto, employees were performing servicing and maintenance on machinery and equipment that included, but were not limited to, Oven 8204 and Vertical Mill 2119. The company did not perform periodic inspections of the lockout/tagout procedures within the last 12 months with all authorized personnel.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: A.) FMC Technologies Measurement Solutions, Inc. - On or about 12/6/18, and at times prior thereto, employees were utilizing lockout procedures for the service of equipment including, but not limited to, Oven 8204. The service on these pieces of equipment included, but were not limited to, repairs and routine preventative maintenance. The lockout procedures utilized were missing information including, but not limited to, identification of all energy sources; the methods used to block mechanical energy sources, and specific steps for testing equipment to verify and determine the effectiveness of lockout devices, tagout devices and other energy control measures.
