29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: A work-related incident which resulted in an employee hospitalization was not reported to OSHA within 24 hours. The incident occurred on or about 11/24/2018 at approximately 11:45 am and was reported to OSHA on 12/04/2018 at approximately 1:18 pm.
