29 CFR 1910.138(a): General requirements. Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. a) LPG container storage: On December 3, 2018, and at times prior, the employer exposed employees to burn hazards while removing and replacing LPG containers on forklifts, in that proper skin protection was not provided.

Paper Cutter Number 2: On or about December 3, 2018, and at times prior, the employer exposed employees to electrical and struck-by hazards in that no specific procedures to control hazardous mechanical energy were developed for servicing such as, but not limited the Champion M.O. Paper Cutter Serial Number 75.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a) Production Area, M.O. Paper Cutter Number 2: On or about November 21, 2018, and at times prior, the employer exposed employees to caught-in hazards in that unguarded sprockets and chains located in the Champion M.O. Paper Cutter Serial Number 75 were not properly guarded.
