29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about the 30-day period of Nov. 8 - Dec. 7, 2018, located throughout a commercial demolition site, employees were performing demolition work near unsupported tilt-up wall panels that had not been instructed on the applicable procedures and/or regulations to minimize those hazards while performing this work and exposed to struck-by hazards.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed. On or about the 30-day period of Nov. 8 - Dec. 7, 2018, employees was engaged in the demolition and removal of up to 18 buildings down to grade at a commercial property. The employer did not ensure that an engineering survey was conducted by a competent person to determine the structural integrity of unsupported tilt-up wall panels, prior to employees performing demolition work, and exposed employees to struck-by hazards.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. On Dec. 7, 2018, an employee was using a skid steer to move demolition debris inside an area of tilt-up wall panels that were not properly supported. The employer did not ensure that continuing inspections were performed by a competent person in that the roof structure of the tilt-up wall panels had been removed, significantly reducing the wall panel's resistance to lateral forces and exposing employees to struck-by hazards.
