29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 12/07/18 in the Fabrication area; employees who changed dies and set up the hydraulic press #911, were exposed to caught between/amputation hazards, when the employer did not require the use of the written control of hazardous energy procedure for this press attached to the back of the press, Revision A, dated 12/15/08.     b) On or about 12/11/18 throughout the facility; employees who performed die set exchanges of hydraulic presses such as but not limited to, MultiPress #5291, #5278, #5279,#5286,#5293, Dennison Multipress #5324, #5070, # 5071, #5008, #435 were exposed to caught between/amputation hazards, when the employer did not require the use of energy control methods.       ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation;     a) On or about 12/07/18 in the Fabrication Department; employees changed the dies on the mechanical power presses, such as but not limited to; Minster Press # 200, 201, 348, 349, 353, Komatsu #207, and were exposed to caught between/amputation or struck by hazards when they were not trained in the methods and means necessary for hazardous energy control before engaging in these types of activities.      b) On or about 12/07/18 throughout the production facility, employees changed the die set on presses such as, but not limited to: Hanna Press #5068, Denison Press # 5070, 5071, Multipress # 5278, 5279, 5286 and were exposed to caught between/amputation or struck by hazards when they were not trained in the methods and means necessary for hazardous energy control before engaging in these type of activities. 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) On or about 12/07/18 in the Fabrication Department; employees were exposed to an amputation hazard when use of the guarding method was not monitored or controlled allowing the use of a foot pedal instead of two hand controls on the hydraulic vertical press #911 (12 ton capacity) permitting access to the point of operation during the operating cycle of this machine.      b) On or about 12/07/18 in the Production area; employees were exposed to crushing injuries when the distance between the palm buttons on pneumatic press # 5160 was 9 inches allowing simultaneous operation of this machine using one hand.       c) On or about 12/07/18 in the Production area ; employees were exposed to crushing injuries when the distance between the palm buttons on pneumatic press # 5323 was 8.75 inches allowing simultaneous operation of this machine using one hand.        d) On or about 12/07/18 at bench number BO3; employees were exposed to crushing injuries when the distance between the palm buttons on welder # 923A was 10 inches allowing simultaneous operation of this machine using one hand.        e)  On or about 12/07/18 at bench number BO3; employees were exposed to crushing injuries when the distance between the palm buttons on welder # 5161 and #5162 was 10 inches allowing simultaneous operation of this machine using one hand.        ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.217(b)(7)(iii): A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) was supplied with the clutch/brake control to select type of operation and the fixing of the selection was not by means capable of supervision by the employer.  a) On or about 03/01/19 in the Fabrication Dept.; employer did not supervise the proper use of settings of mechanical power presses, such as but not limited to, Minster # 349, 348, 353, 200, 201, when the keyed selection switch for the stroke setting of theses presses was found with the key in the switch on each press.    ABATEMENT CERTIFICATION REQUIRED
