29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) On or about December 2, 2018, and at times prior thereto, a crew of Ginno Steel employees were directed to perform a re-roofing job on a low slope (4:12 pitch) roof that included tear-off of older material and installation of new metal roofing materials. Six employees were exposed to multiple fall hazards ranging from heights of 14-21 feet above the ground to wet, slippery new metal roofing materials. As the competent persons, the Superintendent/Lead and/or Project Manager did not identify fall hazards prior to the work beginning. Note: Abatement certification AND supporting documentation are required for this item.

Six employees were exposed to multiple fall hazards ranging from heights of 14-21 feet above the ground to wet, slippery new metal roofing materials. The workers were not provided direction on how to appropriately address the fall hazards for this job. Note: Abatement certification AND supporting documentation are required for this item.
