29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Job site, 7225 McKnight Road, Pittsburgh, PA: On or about December 7, 2018, during construction activities, the employer did not ensure that stairs or a ladder were provided and used to navigate differences in roof elevations of approximately 27 inches that existed on the main roof of the facility.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than what they were designed for: a) Job site, 7225 McKnight Road, Pittsburgh, PA: On or about December 7, 2018, during construction activities, the employer did not ensure that step ladders were used safely in that to gain access to/egress from the main roof of the building to the front roof area, employees climbed sideways onto stepladders that were not secured to prevent movement.
