29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:       a) On December 10, 2018, employees were exposed to eye injury while using hand tools without eye protection during roofing work.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On December 10, 2018, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 22 feet above the ground.    WM Roofing & Masonry & More, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 967089, citation number 2, item number 1 and was affirmed as a final order on February 19, 2015, with respect to a workplace located at 4432 Arthur Ave., Brookfield, Illinois 60513.    No abatement certification or documentation is required for this item.
