29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On April 23, 2026, employees were exposed to eye injury while operating pneumatic roofing nail guns without eye protection during roofing work.  No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On April 23, 2026, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of up to approximately 12.5 feet above the ground.  No abatement certification or documentation is required for this item.  WM Roofing & Masonry & More, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1705577, citation 1, item 1, and was affirmed as a final order on April 18, 2024, with respect to a worksite located at 6359 W. Hyacinth Street, Chicago, IL 60646.
