29 CFR 1926.251(a)(1): Defective rigging equipment was not removed from service:    The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about December 7, 2018 at the establishment located at 7436 1000 Road, in Fredonia, Kansas. Employees were exposed to struck-by hazards in that rigging equipment, a 20 ft. two-leg wire rope sling, used for lifting trusses was not removed from service after deficiencies were discovered.  Deficiencies included nails in the eyes of the safety latches causing the latches to be partially off and open, a kink in the wire rope, exposure of a ribbon, and a broken crown wire.

29 CFR 1926.1412(e)(3)(i)(B): The monthly crane inspection did not include the name and signature of the person who conducted the inspection and the date.    The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about 12/07/18 at the establishment located at 7436 1000 Road, at Fredonia, KS. The employees were exposed in that Terex Crane (Model HC165, Serial-AC4221) used for raising trusses was not inspected monthly. The following deficiencies were observed at the time of the inspection:    a) Wire rope     b) Bent lacing     c) Tracks     d) Hook and latches.

The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about December 7, 2018 at the establishment located at 7436 1000 Road, in Fredonia, Kansas. Employees were exposed to struck-by hazards in that the Terex Crane (Model HC165, Serial-AC4221) used for raising trusses was found to have 2 bent lacings painted blue in the lattice, documented as being deficient, but were not monitored or checked during monthly inspections.

This was most recently documented on or about December 7, 2018 at the establishment located at 7436 1000 Road, in Fredonia, Kansas. Employees were exposed to struck-by hazards in that the annual inspection report, prepared by the employers selected crane inspector, did not:    a)  Clearly document whether the surface of the entire length of the wire rope was inspected, including sections of the wire rope normally hidden during shift and monthly inspections    b)  Did not contain the signature of the person who conducted the inspection
