Employees engaged in commercial roofing activities used the upper half of extension ladders that were secured by rope to the ridge as a means of climbing on a steep pitch roof. Employees engaged in commercial roofing activities of a steep pitch (12/12) roof worked without attaching a restraint lanyard to their fall harnesses. Violation observed on or about December 10, 2018 Hannsy's Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 992257, citation number 1, item number 1, and was affirmed as a final order on September 9, 2014, with respect to a workplace located at 235 Main St. Madison, NJ.
