29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On Dec. 11, 2018, there were no frequent and regular inspections being performed by a competent person in that: (a) employees were utilizing a portable extension ladder on a windy day, to access the second story roof from the porch roof, that had not been secured to the structure and was subsequently blown off the porch roof by a gust of wind (b) employees were 'leapfrogging' the only portable extension ladder on-site on a windy day, pulling up the ladder to the porch roof and then ascending to the next roof line without properly securing the ladder to the structure (c) employees were working on a low-slope pitch residential roof at fall heights greater than 19 feet above the ground below without a fall protection system (d) employees performing roof felting operations did not have fall protection gear on-site before work began at the jobsite

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On Dec. 11, 2018, located throughout a residential roof, employees were performing roofing felt operations on a low-slope pitch roof with unprotected sides and edges while not utilizing a fall protection system. Potential fall heights were measured to be greater than 19 feet above the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. The employer did not provide a training program for employees performing roof felting operations on a low-slope pitch residential roof while not utilizing a fall protection system and exposed to fall heights greater than 19 feet above the ground below.
