29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) West Side Bearing Wall - An employee was standing on a tubular welded frame scaffold work platform while pouring mortar into the top of a block wall and the platform was not fully planked, on or about 12/11/2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) West Side Bearing Wall - An employee was performing work while standing on a tubular welded frame scaffold and the retractable lanyard was stretched out 14 feet way from the anchor point, exposing the employee to a 12 feet fall hazard to a plywood deck below, on or about 12/11/2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
