29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a) Trench - Employees working in the trench greater than 4 feet deep were exposed to cave in hazards from not having a means of egress located in the trench, on or about November 30th, 2018.    Glenn O Hawbaker Inc was previously cited for a violation of this occupational safety and health standard 1926.651(c)(2), which was contained in OSHA inspection number 1081630, citation number 1, item number 1a and was affirmed as a final order on 12/04/15,  with respect to a workplace located at 1612 N Atherton Street, State College, PA 16803.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  ------------------------------------------------------------------------------------------------------------------------  2/19/19 - Standard and AVD changed via Informal Settlement Agreement to the following:      29 CFR 1926.1051(a):     A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.      AVD changed to: a) N. Atherton Street  Employees were exposed to a break in elevation greater than 19 inches on or about November 30, 2018.
