29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: On or about December 12, 2019, in a northeast corner room of a residence under construction located at 120 SW N. Danville Circle in Port Saint Lucie, Florida, an employee performing air conditioning/heating flexible duct work installation was exposed to a fall hazard of approximately 7 feet when standing on the top step of an 8-foot fiberglass a-frame ladder.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: On or about December 12, 2019, in a northeast corner room of a residence under construction located at 120 SW N. Danville Circle in Port Saint Lucie, Florida, employees were exposed to a fall hazard when using ladders to perform air conditioning/heating flexible duct work installation prior to being provided with a training program that would enable them to recognize the hazards associated with ladders.
