29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  Location: 74th King St. New York NY  On or about 12 Dec 2018  a) Employees were decking the floor and were not using any form of fall protection while exposed to the building edge exposing them to more than a 50 foot fall.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Location: 74th King St. New York NY  On or about 12 Dec 2018  a) Employees were decking the floor using personal fall arrest systems; harnesses, retractable lanyards and slings, that were attached to the joist and shores.  Employees would strike the floor the way the fall arrest system was being used.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  Location: 74th King St. New York NY  On or about 12 Dec 2018  a) Employees were not trained in the use of a personal fall arrest system being used to when exposed to falls.  Employees were not anchored and were using the personal fall arrest system in a manner that they would strike the floor below or fall more than 6 feet.
