29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about 12/12/18 at 4695 Radford Road, Flowery Branch, GA: The employer failed to report to OSHA a work-related hospitalization and amputation within 24 hours of such knowledge and did not contact OSHA of the following injury: 11/30/18, Raymond Dalton, the Yard Dog/Spotter Truck was hospitalized with chest and upper body injuries. OSHA became aware of this employees injuries during the inspection opened on 12/10/2018.
